I like to find out why my students are interested in playing music. This helps me to create a lesson plan for each individual one. I like to start out teaching using Alfred's beginner books, and progress from there. Each student learns at a different pace, so I base my lessons off of their progress. My approach varies depending on the student's skill level and chosen instrument, but for beginners I always try to get them familiarized with their instrument: teaching them the basics of reading music and how to navigate the different keys (or strings, etc.). From there I start teaching various musical concepts, the rudiments of music theory, and begin introducing them to different styles of music. When the student is advanced enough, I start having them run through my own custom designed technique and skill drills, and have them start selecting music that they would like to perform on their own.  In essence it helps to build basic musical vocabulary using skills they already have, but may not know that they have them.  For intermediate, advanced, and older students in general I have a more flexible approach that is based upon their goals or lack thereof.  For all students I always emphasize knowledge of music theory and real life application of their musical education toward realizing their own musical goal or dream, as humble or as grand as it may be.
